The HSNNY Team
Each HSNNY customer is assigned an experienced, caring Registered Nurse
Case Manager who specializes in your specific needs. RN Case Managers conduct patient and environmental assessments and coordinated care between all health care team members, including the physician. They monitor vital signs, administer medication, perform injections and IV infusions, change dressings, and much more. Their care enables you to remain in the comfort of your own home.
Physical Therapists facilitate rehabilitation from surgery, illness, or injury. Services include physical therapy, occupational therapy, and speech therapy.
Medical Social Workers connect patients with community resources, assess eligibility for various assistance programs, aid in long-term care planning, and provide counseling and crisis intervention.
Home Health Care Assistants and Personal Care Assistants help patients with the activities of daily living, including: bathing, dressing, light housekeeping, errands, and meal preparation. They can also provide respite services and rehabilitation exercise assistance.
These services are offered in coordination with the patient's family
and private physician, case managers, discharge planners, and trust
officers.
