Case Description:
My client had been admitted for an extended hospital stay because he experienced weakness and shortness of breath. Prior to being hospitalized, my client had been ambulating without assistive devices and was able to enter the bathtub with minimal assistance, but he returned home using a walker and was not able to enter the bathtub at all. His symptoms of dementia had worsened while he was in the hospital, and he was not able to simply lift a fork to his mouth to feed himself upon his return home.
If you are recovering from a long term illness or an extended hospital stay and feel weak and frail, our nurses can provide the following services to help you recover at home and restore your wellness:
- Customize a unique health recovery plan
- Assess home safety
- Assess adaptive equipment and assist with obtaining adaptive equipment
- Assess wounds and change dressings
- Pain management
- Medications and treatment management
- Perform Lab draws and management of anticoagulation (blood thinning)
- Teach family members to properly care for the client
- Act as a liaison between the client’s physician and the family
Our Home Health Aides can assist you with the following daily living activities:
- Personal Care
- Bathing
- Grooming
- Dressing
- Applying medical wear such as braces
- Physical Therapy exercises
- Housekeeping
- Laundry services
- Errands
- Meal preparation
- Medication intake
If your needs exceed the limited availability of a Home Health Aide, our Personal Care Aides can provide additional services to assist you with your daily living activities.
Our Medical Social Workers can:
- Assist with long-term care planning
- Assist with obtaining payment help and applying for programs
- Assist with managing finances and managing resources
- Assist with process of using healthcare proxies
Physical, Speech, and Occupational Therapy as well as Nutrition consultation are available if these services are appropriate for you.
