Case Description:
My client is an elderly woman whom I have been visiting for nearly a year now, and I have been able to prevent her from being placed in a nursing home by providing her with home care services. I make weekly visits to pour her medications and ensure she is complying with her medical care needs because she is forgetful and her healthcare routines must be monitored. I have also provided SN intervention to prevent altered skin integrity and altered CP status.
