Case Study: Nursing Home Placement Prevention

Patient Condition

My client is an elderly woman whom I have been visiting for nearly a year now, and I have been able to prevent her from being placed in a nursing home by providing her with home care services. I make weekly visits to pour her medications and ensure she is complying with her medical care needs because she is forgetful and her healthcare routines must be monitored. I have also provided skilled nursing intervention to prevent altered skin integrity and altered chronic pancreatitis status.

Patient Treatment

Our Home Health Aides have provided services to assist her with meal preparation, application of compression stockings to prevent edema and return of open areas, as well as personal care and homemaking. Physical therapy services help my client to prevent falls and improve patients strength and safety.

Patient Recovery

Altogether, these services have prevented my client from entering a nursing home in the past year: we help to keep her health from deteriorating and allow her to retain her independence and live in her home. My client’s daughter is very grateful for our services and very satisfied with the consistency and regularity of our healthcare providers who work to keep her mother in the comfort of her own home.
