Our Community Liaison will maintain and develop efficient working relationships between our home health care agency and community referral sources, including doctors, discharge planners, and other organizations. S/he will be an astute problem-solver who listens, understands, and acts on the needs of referral sources. 

Hours are flexible (16-24pw), but must be fulfilled between 7:30AM and 4PM.  Health care background and knowledge of community resources preferred; computer proficiency necessary; must enjoy being outside the office and interacting with medical professionals.

Our new Community Liaison must:

• Have the self-confidence to speak intelligently about our product to discharge planners, doctors, and other medical professionals
• Have the self-discipline and motivation necessary to work outside the office, without strict supervision
• Have a professional appearance & demeanor
• Be out-going and enjoy meeting new people
• Not be intimidated by anyone
• Consult with co-workers on the best strategies and methods for achieving common goals
• Assist co-workers in the performance of their jobs, even when there is no direct benefit to you

Responsibilities include:

• Increasing referral volume
• Serving co-workers and referral sources with enthusiasm and dedication
• Using community resources and social agencies to promote HSNNY 
• Developing and maintaining a successful community awareness program 
• Completing a market analysis of competitors

Requirements:

• Health Care background or experience is an advantage
• Knowledge of community resources and social agencies an advantage
• Computer Proficiency, Word, Excel, Power Point, and Internet Explorer

RN Case Manager

Our home health RN Case Managers manage post-operative, acute, and custodial care cases.  They perform assessments, institute care plans according to doctors’ orders, and ensure that case staff follow through on home care plans. Our two current opportunities are for the Massena and Canton/Ogdensburg areas.

At least one year of previous nursing experience required; valid NY State RN license; an extraordinary ability to build relationships with customers, co-workers, and medical professionals.

RN Care Supervisor

We seek an RN licensed and registered in NYS to supervise personal care aides in Waddington, Ogdensburg, and Morristown.  As the junior partner of the RN Case Manager, this individual will orient, schedule, and supervise personal care aides.  S/he will ensure that aides execute care plans and provide superior, dependable service to our clients.

The Health Care Supervisor must enjoy building personal relationships with customers and co-workers.  Excellent organizational skills will be necessary to coordinate and supervise aides in multiple locations. 

The job is an excellent stepping stone to the position of RN Case Manager.

Duties:

• Ensuring that aides carry out care plans (developed by the RN Case Manager) and provide excellent service
• Aide orientation
• Supervision of aides and unannounced supervisory visits to clients’ homes while aides are on duty
• Responding to client concerns

Requirements:

• Must be licensed and registered as an RN in NY state
• At least one year of nursing experience required
• Superb organizational skills
• Ability to lead staff by professional example
• Bachelor degree an advantage, but not necessary
• Supervisory or home health care experience  are advantages, but on necessary
